Issued by [place and date of issue]                                                     and coming from the                                           faith, having by Divine Grace, voluntarily and in all consciousness, joined the Orthodox, that is, the One Holy Catholic and Apostolic Church, give this day, in the Holy Metropolitan diocese of                                 , this present Libel document, whereby on the one hand I deny, reject and renounce unreservedly everything that is of the                                     faith and on the other hand, confess and agree with everything that is of the Holy Seven Ecumenical and the Local Councils, of the declarations and their consenting Doctrines. More specifically I believe that:

 1) God is Triune.

 2) Jesus Christ is consubstantial with the Father and truly God.

 3) Jesus Christ is equal to the Father, without beginning, eternal, begotten and not

     created, King and Lord, the Son of God.

 4) The Holy Spirit is the third person of the Holy Trinity and proceeds only from the

      Father.

 5) The soul is immortal and the bodies will be resurrected with the Second Coming of

      Christ.

 6) The kingdom of God is heavenly and eternal.

 7) Hell is eternal.

 8) The Second Coming of the Lord is visible and unknown as to when it will be

       realized.

 9) The Mysteries (Sacraments) of the Church are: a) Baptism, b) Chrismation, c) The

      Holy Eucharist, d) Confession, e) The Priesthood, f) Matrimony, and g) Holy

      Unction.

10) Holy Tradition is a second source of faith equal in authority with Holy Scripture.

11) I impart the obligatory honorary veneration to the Icons portraying the persons of

       Christ, the Most Holy Mother of God, and the Saints.

12) I make the sign of the Life-giving Cross. I confess and wholeheartedly embrace the

      Orthodox and pledge that I will be a faithful and true child of Her Church.
